You can register ONLINE by clicking here You will be required to submit your correspondence details and to indicate any special requirements you may have. Be sure to select the correct type of registration: Standard Registration, Community Registration or BHIVA RegistrationScholarship. If required, be sure to select the correct registration fee and, if necessary, please enter your membership number. Please indicate which social events you wish to attend. Please make your payment by credit or debit card.

You will receive an automated confirmation of your registration via email.

Alternatively, please download and complete the registration form, and return it with your payment to the Conference Organisers.

Cancellation
Providing that written notice of cancellation is received by 5 September 2008, 75% of the registration fee will be returned. If the conference cannot be held for any reason, the Conference Organiser’s liability will be limited to a refund of the registration fee only. No refunds will be made after this date; although it will be possible to substitute a delegate of the SAME FEE CATEGORY up to the day of the conference.

Continuing Professional Development (CPD)
Medical staff in career grade posts who are enrolled with one of the Royal Medical Colleges for Continuing Professional Development will receive CPD credits at the rate of one CPD credit per conference hour (exclusive of travel, refreshments, social events and satellite symposia). The conference will be allocated a maximum of 6 CPD credits per conference day.

BHIVA lunchtime workshops
Places are limited and will be restricted to the first 70 applicants. Please only register if you intend to come. If subsequently you discover you cannot attend any of the lunchtime workshops, please let the Conference Organisers know at your earliest convenience.


Social events
A drinks reception has been arranged after close of sessions on Thursday 9 October at 1830.
