Electronic Personal Health Records (PHR) Survey

The Royal College of Physicians (RCP) Health Informatics Unit (HIU) is undertaking a “landscape review” of current electronic personal health and care record (PHR) activity in the UK with assistance from a grant from NHS England.

For this review, a PHR is defined as: "a digital tool that helps people to maintain their health and manage their care. It may do this by enabling them to capture their own health and care data, to communicate with health and care services, and/or to have access to their care record."

Whether you are a member of the public who uses a PHR, a health/care professional PHR user, a PHR supplier or have implemented (or have plans to implement) a PHR, we would like to hear from you. So, we would be extremely grateful if you could complete our short, confidential online survey at https://www.surveymonkey.com/r/RLW8W6N

This should take no more than 20 minutes and the survey will close at 5pm on 13 September 2015.
